SURG-22. STAGED LASER INTERSTITIAL THERAPY (LITT) FOR THE SURGICAL TREATMENT OF INSULAR GLIOMA: A CASE SERIES WITH A NOVEL TREATMENT PARADIGM

Abstract BACKGROUND Insular gliomas pose a significant surgical challenge due to the insula’s delicate surrounding functional and vascular anatomy. are conventionally treated with open resection augmented by several adjuvants. Despite advancements in technology techniques, of insular has been associated frequent potentially severe morbidity. Laser interstitial thermal therapy (LITT) is novel, cytoreductive, less-invasive treatment option for glioma. OBJECTIVE We describe first-ever series patients glioma staged LITT or without subsequent craniotomy. METHODS A retrospective institutional database identified who underwent treatments. Clinical, histopathological, volumetric lesional characteristics were obtained. Procedural characteristics, morbidity, overall survival (OS), progression-free (PFS) determined. RESULTS Between 2019-2022, 11 stages total 21 procedures Four had Diffuse Astrocytoma (IDHmt/TP53+), one Gemistocytic three Anaplastic Oligodendroglioma (IDHmt/1p19q codel), secondary GBM (IDHwt/TP53-). Six alone, while five followed craniotomy complete resection. Patients median tumor volume 31.5 cm3 (9.58, 97) mean extent ablation±resection 97.68% (S.D. 5.4). At follow-up 12.59 months, PFS 12.88 months an OS 14.46 months. No change KPS following LITT. Two decrease surgery.
